Begin by completing our Patient Referral Form. Once received, we’ll promptly begin assigning a qualified wound care provider.
To help us move quickly, please include key details such as wound photos, H&P, demographics, and insurance information.

Step 2:
Progress Alerts
We keep you informed every step of the way. Choose your preferred method of communication—call, text, or email—for status updates.
We’re also developing a secure portal where you’ll soon be able to track referral progress in real-time as providers complete each task.
Step 3:
Care Coordination Through Completion
We continue to monitor each referral through the entire care journey. You’ll be notified at every important milestone:
